Regulation No. 23/2020 issued by the Rector of the Wroclaw University of Environmental and Life Sciences on the 23 rd of January 2020

on the rules of preparing and issuing graduation diplomas and the supplement to the aforementioned diplomas issued to graduates of the Wroclaw University of Environmental and Life Sciences

Based on the Art. 23 par. 2 point 2 in connection with the Art. 77 par. 1-3 of the Act issued on the 20th of July 2018 - Law on Higher Education and Science (Journal of Laws of 2018, item 1668, as amended), in connection with the Art. 265 point 2) of the Act issued on the 3rd of July 2018 Regulations Introducing the Act - Law on Higher Education and Science (Journal of Laws of 2018, item 1669, as amended), as well as in connection with § 22 and 23 and § 32-36 of the Regulation of the Ministry of Science and Higher Education issued on the 27th of September 2018 on studies (Journal of Laws of 2018, item 1861 as amended) with Resolution No. 87/2019 adopted by the Senate of the Wroclaw University of Environmental and Life Sciences on the 25th of September 2019 on the approval of the model graduation diplomas from the Wroclaw University of Environmental and Life Sciences and Resolution No. 107/2019 adopted by the Senate of the Wroclaw University of Environmental and Life Sciences on the 25th of October 2019 on the approval of the model of joint studies diploma concerning the MA/MSc studies at the Wroclaw University of Environmental and Life Sciences, it was decided as follows:

§ 1 

1. The model graduation diploma issued by the Wroclaw University of Environmental and Life Sciences is determined in the Resolution No. 87/2019 adopted by the Senate of the University on the 25th of September 2019 on the approval of the model graduation diplomas issued by the Wroclaw University of Environmental and Life Sciences.

2. The model diploma confirming the completion of joint MA/MSc studies at the Wroclaw University of Environmental and Life Sciences is presented the in Resolution No. 107/2019 adopted by the Senate of the University on the 25th of October 2019 on the approval of the model diploma concerning the completion of joint MA/MSc studies at the Wroclaw University of Environmental and Life Sciences.

3. The model supplement is indicated in the regulation issued by the Ministry of Science and Higher Education on studies.

§ 2 

1. The University prepares so-called set of documents confirming graduation within 30 days from the date of graduation. The aforementioned set includes:

1) original diploma;

2) two copies of the diploma in Polish;

3) a copy of the diploma in Polish to be stored in the file;

4) the original of the supplement;

5) two copies of the supplement in Polish;

6) a copy of the supplement in Polish o be store in the file.

2. The student has to provide the information necessary to prepare the Diploma Supplement together with the documents confirming such information no later than on the day preceding the diploma examination. In the case of veterinary medicine, the aforementioned documents are to be provided 10 days prior to entering the final exam.

3. Prior to graduation, the student can file a written application for the issuance of:

1) copy of the diploma in English - instead of one copy of the diploma in Polish;

2) one copy of the diploma supplement in English - instead of one copy of the supplement in Polish.

4, Wroclaw University of Environmental and Life Sciences issues an additional copy of the graduation diploma and the Diploma Supplement translated into English based on the graduate's request.

5. The documents indicated in par. 4, are issued within 30 days from the date of filing the application, after prior payment of the fee by the graduate for the issuance of an additional copy of the aforementioned documents.


6. The University charges fees for the issuance of additional copies pursuant to the Art. 79 par. 2 point 4 of the Act - Law on Higher Education and Science.

7. In case of students admitted to studies prior to 2019/2020 academic year:

1) prior to graduation, the University charges the student with a fee following the provisions of the agreement concluded pursuant to the Art. 160a of the Act - Law on Higher Education for the issuance of:

    a) the original graduation diploma with two copies in Polish - in the amount of PLN 60,

    b) a copy of the graduation diploma translated into English - in the amount of PLN 40.

2) The University does not charge a fee for issuing the original Diploma Supplement and its copies in Polish.

8. In case of students admitted to studies from 2019/2020 academic year:

1) The University does not charge a fee for the issuance of the diploma and supplement and their two copies, this include, a copy in English, if the student submitted the application indicated in section 3;

9. The University charges a fee only for the issuance of a copy of the diploma and the diploma supplement in English other than indicated in par. 3 - in the amount of PLN 20.

§ 3 

1. The diploma and supplement as well as their copies are provided to the graduate in person or to an authorized person upon  the graduate’s written request.

2. The authorization referred to in sec. 1 is to contain confirmation concerning the authenticity of the signature of the authorizing person made in the presence of an employee authorized to issue documents or confirmed by a notary.

3. The graduate confirms receipt of the documents indicated in par. 1 by signing the form titled “Confirmation of receiving the documents”. The aforementioned form is kept in the student's personal file.

4. The person employed in the dean's office is obliged to make appropriate individual markings on the form titled “Confirmation of receiving the documents” using the technique of typography assigned to individual forms of diplomas (for example, AA 0000000), used to prepare the diploma confirming the completion of studies and its copies for a given.

5. Graduate-related documents listed in par. 1 are equipped with the cover prepared observing the University's Visual Identification System.

§ 4 

1. In case of loss of the original diploma or diploma supplement, the graduate has the possibility of filing a written request for duplicates thereof. The reasons for the loss of the aforementioned documents have to be indicated.

2. A duplicate is issued based on the documents held, in particular based on a copy of the diploma or a supplement included in the student's personal file.

3. A duplicate shall be made on the form of the diploma or the diploma supplement, according to the model applicable on the date of issuing the original documents. 

4. There is an inscription “DUPLICATE” and the date of issuing the duplicate according to the formula: “DUPLICATE was issued on (day-month-year)” on the diploma printout, in the upper right corner.

5. When it comes to the diploma supplement printout, the word “DUPLICATE” shall be placed above the words “DIPLOMA SUPPLEMENT” together with the date of issue following the formula “DUPLICATE issued on (day-month-year)”.

6. The duplicate of the diploma does not include a photo (if the photo constituted a necessary element of the diploma), the space designated for the photo should be crossed out.

7. The names and surnames of the persons who signed the original diploma together with information concerning their titles and academic degrees are placed in the places reserved for stamps and signatures.

8. The duplicate of the diploma and the supplement is signed by the current rector or an authorized person holding a managerial position at the university.

9. If there are no forms of the diploma or the diploma supplement in accordance with the model applicable on the date of issuing the original document, a duplicate shall be issued using a form prepared by the University. This form must be consistent with the content of the original diploma or the original diploma supplement.

10. Information concerning the issuance of a duplicate of the diploma confirming the completion of studies or the diploma supplement shall be included in the student's personal file together with the confirmation of its receipt.

11. Information concerning the issuance of a duplicate of the graduation diploma is to be recorded in the book of diplomas and in USOS tab “Diplomas - diplomas and exams” in the “comment” field.

12. The duplicate of the diploma and the Diploma Supplement are perceived as the original and can be reissued in case of loss.

13. When it comes to the receipt of a duplicate of the graduation diploma or the diploma supplement, § 3 par. 1 and 2 shall apply accordingly.

§ 5

1. A diploma, a supplement to the diploma and their copies with errors or mistakes are to be replaced ex officio or upon a written request of the graduate.

2. The rules and procedure provided for in this ordinance shall apply accordingly to the exchange of the diploma of completion of studies, the diploma supplement and their copies.

3. The new diploma confirming the completion of studies, the diploma supplement and their copies are issued under the current date and signed by the current rector or an authorized person holding a managerial position at the University.

4. The issuance of a new diploma or supplement to the diploma requires the return of the exchangeable diploma or supplement with copies.

5. No fee is charged for replacing the diploma or supplement issued with errors or mistakes not attributable to the graduate.

§ 6

1. After graduation, the first name (s) or surname of the graduate shall not be changed when it comes to his or her graduation diploma and in the supplement, subject to par. 2-4.

2. Changing the name (s) and surname on the graduation diploma and the diploma supplement after graduation is allowed only in the following circumstances:

1) an administrative decision issued by the head of the registry office;

2) a court ruling completing gender reassignment proceedings.

3. In the case referred to in sec. 2, the graduate is obliged file a written application addressed to the relevant dean's office. The application is to indicate the document constituting the grounds for the change of personal data together along with information concerning the authority that issued it, the date of its issuance and the number, if any. Such document is to be presented for inspection.

4. In case of a change of the first name (s) or surname of the graduate, referred to in par. 2:

1) a new diploma confirming the completion of studies and a diploma supplement are issued using a new name (s) and surname, upon the graduate's written request and upon presentation of an administrative decision or a court ruling;

2) a new graduation diploma and the diploma supplement are drafted using the current date and are signed by the current rector or an authorized person holding a managerial position at the university;

3) the issuance of a new diploma confirming the completion of studies and the diploma supplement are to be realized provided the complete diploma and supplement together with copies and  issued under the previous name (s) or surname are returned.

5. If the name (s) or surname is changed prior to graduation, the diploma and supplement with copies are issued using the new name (s) or surname. The new name (s) or surname may be entered only based on a copy of the marriage certificate, an administrative decision on changing the name (s) or surname or a court decision.

§ 7 

1. The rules for preparing a graduation diploma are determined in Attachment 1 to this regulation.

2. The rules for preparing the Diploma Supplement are determined in Attachment 2 to this regulation.

3. Detailed rules for making printouts of graduation diplomas are determined in the Attachment 3 to this regulation.

§ 8 

1. The deans of the faculties are responsible for execution of this regulation.

2. The vice-rector for student affairs and education is responsible for supervision over the implementation of the regulation.

§ 9

The Regulation No. 167/2017 issued by the Rector of the Wroclaw University of Environmental and Life Sciences on the 6th of November 2017 on rules of preparing and issuing higher education diplomas and the diploma supplements issued to graduates of the Wroclaw University of Environmental and Life Sciences is repealed.

§ 10

The regulation comes into force on the day it is signed.
